Bring Better to Value-Based Care as the Director of Clinical Quality, ACO

We believe in bringing "better" to our patients, partners, and communities through innovative care models and a deep commitment to quality outcomes. At Sound Physicians, we are transforming healthcare through value-based care initiatives that improve quality, strengthen care coordination, and reduce total cost of care.

We are seeking a Director of Clinical Quality (RN) to provide clinical leadership and strategic direction for an assigned portfolio of Accountable Care Organization (ACO) partnerships. This role is accountable for driving clinical strategy, quality performance, care model integrity, and population health outcomes across participating providers and facilities.

Qualifications:

Active, unrestricted RN license in the state of residence.

Bachelor's degree in nursing (BSN) or equivalent combination of education and experience.

6+ years of progressive nursing experience, including at least 2 years in post-acute care, skilled nursing, assisted living, independent living, home-based primary care, care coordination, population health, or a related setting serving high-needs Medicare beneficiaries.

Demonstrated experience leading complex, cross-functional initiatives through influence rather than direct authority.

Preferred: Prior nursing leadership experience and/or experience in quality improvement, transitional care, case management, or value-based care.

Authorized to work in the United States.

Scheduling:

Full-time position. Monday through Friday business hours.

Flexible work environment with collaboration across multiple markets and partner organizations.

Periodic travel may be required to support partner engagement and business initiatives.

Key Responsibilities:

Provide clinical leadership and strategic direction for ACO clinical initiatives, leading the development and optimization of workflows that improve quality outcomes, reduce avoidable utilization, strengthen care transitions, and advance value-based care performance.

Serve as the primary clinical relationship lead for participating providers and facilities, partnering with ACO operations, clinical transformation, network performance, and executive leadership to drive clinical performance and adoption of ACO priorities.

Drive population health and care coordination strategies focused on high-needs Medicare beneficiaries, including reducing avoidable hospitalizations and readmissions, expanding appropriate virtual care, increasing preventive care and Annual Wellness Visit completion, closing quality gaps, and supporting advance care planning.

Leverage quality, utilization, and population health data to identify performance opportunities, establish clinical priorities, and lead improvement strategies that advance quality measures, accurate documentation, beneficiary engagement, and total cost-of-care objectives.

Lead clinical performance discussions within Monthly Performance Reviews (MPRs), Joint Operating Committees (JOCs), and other executive governance forums. Communicate performance trends, emerging risks, and strategic recommendations while influencing partner leadership to advance evidence-based practices and care delivery transformation.

Provide clinical direction, coaching, and dotted-line leadership to nursing and other clinical resources supporting assigned partnerships, ensuring alignment and consistent execution of ACO clinical priorities.

Knowledge, Skills & Abilities:

Strong clinical knowledge of high-needs Medicare beneficiaries and care delivery across post-acute and community-based settings.

Understanding of value-based care, ACO operations, Medicare quality programs, and population health management.

Experience interpreting clinical performance data and leading quality improvement strategies to reduce avoidable utilization and improve patient outcomes.

Ability to influence and collaborate with physicians, advanced practice clinicians, facility leaders, and executive stakeholders.

Excellent communication, presentation, organizational, and problem-solving skills with the ability to manage competing priorities independently.

Demonstrated professionalism, discretion, and sound judgment when managing sensitive situations and confidential information.

Rewards and Compensation:

Competitive base salary ranging from $115,000 to $125,000 + annual incentive opportunity.

Comprehensive health, dental, vision, and retirement benefits.

Professional development and leadership growth opportunities.

Additional robust benefits plan available through Sound Physicians.
